| AMENDMENT TO HOUSE BILL 4054

| AMENDMENT NO. ______. Amend House Bill 4054 on page 18, by |

| replacing lines 17 through 23 with the following: |

| "yet attained the age of 21. The Department shall have |
| 5 |
| responsibility for the development and delivery of services |
| 6 |
| under this Section. An eligible youth may access services under |
| 7 |
| this Section through the Department of Children and Family |
| 8 |
| Services or by referral from the Department of Human Services. |
| 9 |
| Youth participating"; and

| 10 |
| on page 19, by replacing lines 8 through 12 with the following: |
| 11 |
| "Children and Family Services shall create clear, readable |
| 12 |
| notice of the rights of former foster youth to child welfare |
| 13 |
| services under this Section and how such services may be |
| 14 |
| obtained. The Department of Children and Family Services and |
| 15 |
| the Department of Human Services shall disseminate this |
| 16 |
| information statewide. The Department shall adopt |
| 17 |
| regulations"; and
