| AMENDMENT NO. ______. Amend House Bill 4088, AS AMENDED, |

| with reference to page and line numbers of Senate Amendment No. |

| 1, by replacing line 15 on page 9 through line 15 on page 10 |

| with the following: |

| "(3) Post upon establishment or revision a notice of |

| the existence and character of the system of records, which |
| 8 |
| notice shall include the following: |

| (A) the name and location of the system; |
| 10 |
| (B) the categories of individuals on whom records |
| 11 |
| are maintained in the system; |
| 12 |
| (C) each routine use of the records contained in |
| 13 |
| the system, including the categories of users and the |
| 14 |
| purpose of such use; |
| 15 |
| (D) the title and business address of the agency |
| 16 |
| official who is responsible for compliance with this |
| 17 |
| Act; |

| (E) the categories of sources of records in the |
