    AMENDMENT NO. ______. Amend House Bill 4088, AS AMENDED,

with reference to page and line numbers of Senate Amendment No.

1, by replacing line 15 on page 9 through line 15 on page 10

with the following:

        "(3) Post upon establishment or revision a notice of 
7
    the existence and character of the system of records, which 
8
    notice shall include the following:

            (A) the name and location of the system;
10
            (B) the categories of individuals on whom records 
11
        are maintained in the system;
12
            (C) each routine use of the records contained in 
13
        the system, including the categories of users and the 
14
        purpose of such use;
15
            (D) the title and business address of the agency 
16
        official who is responsible for compliance with this 
17
        Act;

            (E) the categories of sources of records in the
