| 1 | AMENDMENT TO HOUSE BILL 4924

| 2 | AMENDMENT NO. ______. Amend House Bill 4924 on page 1, by | ||||||
| 3 | replacing line 11 through line 14 with the following: | ||||||
| 4 | "96th General Assembly must provide coverage for an operation, | ||||||
| 5 | either monaurally or binaurally, to implant cochlear implants | ||||||
| 6 | and cochlear devices, including all internal and external | ||||||
| 7 | components, and provide post-treatment services, including, | ||||||
| 8 | but not limited to, programming, troubleshooting, repairs, | ||||||
| 9 | replacement components, such as external speech processors, | ||||||
| 10 | microphones, coils, headsets, cables, and batteries, FM | ||||||
| 11 | systems, auditory training, aural rehabilitation, and speech | ||||||
| 12 | therapy for children identified by 18 years of age as having | ||||||
| 13 | hearing impairment to the degree that they would benefit from | ||||||
| 14 | implantation. These services must be provided by a speech | ||||||
| 15 | pathologist, audiologist, or physician licensed to practice in | ||||||
| 16 | this State.".
