| AMENDMENT NO. ______. Amend House Bill 5448, AS AMENDED, in |

| Section 5, by replacing the last paragraph of Sec. 203 with the |

| following:

| "With each marriage license, the county clerk shall provide |
| 6 |
| a pamphlet
describing the causes and effects of fetal alcohol |
| 7 |
| syndrome. At least annually, the county board shall cause to be |
| 8 |
| made an audit of the county clerk's compliance with the |
| 9 |
| requirement that the county clerk provide a pamphlet with each |
| 10 |
| marriage license. The county board shall cause each audit to be |
| 11 |
| filed with the Illinois Department of Public Health. All |
| 12 |
| funding and production costs for the aforementioned |
| 13 |
| educational pamphlets for distribution to each county clerk |
| 14 |
| shall be provided by non-profit, non-sectarian statewide |
| 15 |
| programs that provide education, advocacy, support, and |
| 16 |
| prevention services pertaining to Fetal Alcohol Syndrome.".
