| AMENDMENT TO HOUSE BILL 5879

| AMENDMENT NO. ______. Amend House Bill 5879, on page 1, |

| line 9, by replacing "a" with "an electric or gas"; and
|

| on page 1, immediately below line 12, by inserting the |
| 5 |
| following:
|

| "Electric and gas utilities shall obtain from customers |
| 7 |
| requesting service switches 2 forms of identification from the |
| 8 |
| following list: (i) a social security number, (ii) a driver's |
| 9 |
| license number and state of issuance, (iii) a state |
| 10 |
| identification card, (iv) a passport, (v) a birth certificate, |
| 11 |
| (vi) immigration and naturalization documents, (vii) a tax |
| 12 |
| identification number, (viii) a business license, (ix) |
| 13 |
| articles of incorporation, or (x) another valid |
| 14 |
| government-issued photo identification.".
