on page 2, line 21, by replacing "(e)" with "(d)"; and

on page 2, line 25, by deleting "is pregnant or"; and

on page 2, line 26, by replacing "(f)" with "(e)"; and

by replacing line 13 on page 3 through line 10 on page 4 with
