AMENDMENT TO HOUSE BILL 6362

    AMENDMENT NO. ______. Amend House Bill 6362 by replacing 
3
everything after the enacting clause with the following:

    "Section 5. The Illinois Income Tax Act is amended  by 
5
adding Section 507UU as follows:
 
6
    (35 ILCS 5/507UU new)
7
    Sec. 507UU. Education, Health, and Human Services 
8
checkoff.  For taxable
years ending on or after December 31, 
9
2010, the Department must
print on its standard individual 
10
income tax form a provision
(i) indicating that if the taxpayer 
11
wishes to contribute to the
Common School Fund and the Health 
12
and Human Services Medicaid Trust Fund, as authorized by this 
13
amendatory Act of
the 96th General Assembly, he or she may do 
14
so by stating the
amount of the contribution (not less than $1) 
15
on the return and
(ii) stating that the contribution will 
16
reduce the taxpayer's refund or
increase the amount of payment

to accompany the return. Moneys received from the this tax 
2
checkoff shall be deposited as follows: 50% into the Common 
3
School Fund and 50% into the Health and Human Services Medicaid 
4
Trust Fund.  Failure
to remit any amount of increased payment 
5
shall reduce the
contribution accordingly. This Section does 
6
not apply to any
amended return.

    Section 99. Effective date. This Act takes effect upon 
8
becoming law.".
