1 | AMENDMENT TO HOUSE BILL 6362

2 | AMENDMENT NO. ______. Amend House Bill 6362 by replacing | ||||||
3 | everything after the enacting clause with the following:

4 | "Section 5. The Illinois Income Tax Act is amended by | ||||||
5 | adding Section 507UU as follows: | ||||||
6 | (35 ILCS 5/507UU new) | ||||||
7 | Sec. 507UU. Education, Health, and Human Services | ||||||
8 | checkoff. For taxable
years ending on or after December 31, | ||||||
9 | 2010, the Department must
print on its standard individual | ||||||
10 | income tax form a provision
(i) indicating that if the taxpayer | ||||||
11 | wishes to contribute to the
Common School Fund and the Health | ||||||
12 | and Human Services Medicaid Trust Fund, as authorized by this | ||||||
13 | amendatory Act of
the 96th General Assembly, he or she may do | ||||||
14 | so by stating the
amount of the contribution (not less than $1) | ||||||
15 | on the return and
(ii) stating that the contribution will | ||||||
16 | reduce the taxpayer's refund or
increase the amount of payment |

1 | to accompany the return. Moneys received from the this tax | ||||||
2 | checkoff shall be deposited as follows: 50% into the Common | ||||||
3 | School Fund and 50% into the Health and Human Services Medicaid | ||||||
4 | Trust Fund. Failure
to remit any amount of increased payment | ||||||
5 | shall reduce the
contribution accordingly. This Section does | ||||||
6 | not apply to any
amended return. | ||||||
7 | Section 99. Effective date. This Act takes effect upon | ||||||
8 | becoming law.".
