AMENDMENT TO SENATE BILL 1617

    AMENDMENT NO. ______. Amend Senate Bill 1617 on page 3,

line 15, after "renewed", by inserting "for an applicant whose 
4
facility is located in Cook County"; and

on page 3, line 22, by replacing "proportions" with 
6
"percentages"; and
 
7
on page 3, by replacing lines 23 through 26 with the following:

    "underserved patients that must be served; and
9
        (ii) submits a notarized statement signed by the 
10
    Chief"; and
 
11
on page 6, line 20, after "installation", by inserting "located 
12
in Cook County"; and
 
13
on page 7, line 1, by replacing "proportions" with 
14
"percentages"; and
