| AN ACT concerning civil law.
|
| 2 |
| Be it enacted by the People of the State of Illinois, |
| 3 |
| represented in the General Assembly:
|
| 4 |
| Section 5. The Illinois Marriage and Dissolution of |
| 5 |
| Marriage Act is amended by changing Section 202 as follows:
|

Marriage License and

| (a) The
Director of Public Health shall prescribe the form |
| 9 |
| for an
application for a marriage license, which shall include |
| 10 |
| the following
information: |
| 11 |
| (1) name, sex, occupation, address, social security |
| 12 |
| number,
date and place of birth of each party to the proposed |
| 13 |
| marriage;
|
| 14 |
| (2) if either party was previously married, his name, and |
| 15 |
| the
date, place and court in which the marriage was dissolved |
| 16 |
| or declared
invalid or the date and place of death of the |
| 17 |
| former spouse;
|
| 18 |
| (3) name and address of the parents or guardian of each |
| 19 |
| party; and
|
| 20 |
| (4) whether the parties are related to each other and, if |
| 21 |
| so,
their relationship.

| (b) The Director of Public Health shall prescribe the forms |

the marriage license, the marriage certificate and, when |
