| AN ACT concerning corrections.

| Be it enacted by the People of the State of Illinois, |
| 3 |
| represented in the General Assembly:
|

| Section 5. The Unified Code of Corrections is amended by |
| 5 |
| changing Section 3-5-2 as follows:
|

| Sec. 3-5-2. Institutional Record. The
The Department shall |

| maintain records of the examination, assignment,
transfer, |

| discipline of committed persons and what grievances, if any, |
| 10 |
| are
made in each of its institutions, facilities and programs. |

| The record shall
contain the name of the persons involved, the |
| 12 |
| time, date, place and purpose
of the procedure, the decision |
| 13 |
| and basis therefor, and any review of the
decision made. |
