| 2 | | AMENDMENT NO. ______. Amend House Bill 1261, AS AMENDED, by |
| 3 | | replacing everything after the enacting clause with the |
| 4 | | following:

| 5 | | "Section 5. The Vital Records Act is amended by changing |
| 6 | | Section 5 as follows:
|
| 7 | | (410 ILCS 535/5) (from Ch. 111 1/2, par. 73-5)
|
| 8 | | Sec. 5. (1) The State Registrar of Vital Records shall:
|
| 9 | | (a) Administer and enforce this Act and the rules and |
| 10 | | regulations issued
hereunder;
|
| 11 | | (b) Direct, supervise, and issue instructions necessary to |
| 12 | | the efficient
administration of a statewide system of vital |
| 13 | | records, the Office of Vital
Records, and be custodian of its |
| 14 | | records;
|
| 15 | | (c) Direct, supervise, and control all activities of local |
| 16 | | and
subregistrars;

| 1 | | (d) Prescribe and distribute such forms as are required by |
| 2 | | this Act and
the rules and regulations issued hereunder;
|
| 3 | | (e) Arrange the certificates in a systematic manner; |
| 4 | | prepare and
maintain a comprehensive index; permanently |
| 5 | | preserve and keep all such
records either in the original or in |
| 6 | | photographic or micro-photographic
form;
|
| 7 | | (f) Prepare and publish reports of vital statistics of this |
| 8 | | State, and
such other reports as may be required by the |
| 9 | | Department.
|
| 10 | | (g) Prepare and transmit monthly a report to each county |
| 11 | | assessor and each mass transit district indicating the name, |
| 12 | | address, date of birth, date of death, and social security |
| 13 | | number of each decedent who resided within the county or mass |
| 14 | | transit district and died within the reporting period. The |
| 15 | | State Registrar of Vital Records shall adopt rules to implement |
| 16 | | this subsection (g). |
| 17 | | (2) The State Registrar of Vital Records may delegate such |
| 18 | | functions and
duties as are necessary to properly carry out the |
| 19 | | provisions of this Act.
