AMENDMENT TO HOUSE BILL 4003 




2    AMENDMENT NO. ______. Amend House Bill 4003 by replacing 
3everything after the enacting clause with the following:

 
4    "Section 5. The Mental Health and Developmental 
5Disabilities Code is amended  by changing Sections 3-201 and 
64-202 as follows:
 



7    (405 ILCS 5/3-201)  (from Ch. 91 1/2, par. 3-201)



8    Sec. 3-201. 
The Department shall prescribe all forms 
9necessary for
proceedings under this Chapter, and all forms 
10used in such proceedings shall
comply substantially with the 
11forms so prescribed.  The Department shall
publish all forms in 
12electronic format and post the forms to its website print and 
13furnish an initial supply of such forms to the clerks of the 
14circuit courts.
