| 2 | | modalities effective in early detection, prevention, cure, |
| 3 | | screening, and treatment of childhood cancer and may include |
| 4 | | clinical trials. The grant funds may not be used for |
| 5 | | institutional overhead costs, indirect costs, other |
| 6 | | organizational levies, or costs of community-based support |
| 7 | | services. |
| 8 | | Section 10. The Illinois Income Tax Act is amended by |
| 9 | | adding Section 507AAA as follows: |

| 11 | | Sec. 507AAA. The Childhood Cancer Research Fund checkoff. |
| 12 | | For taxable years ending on or after December 31, 2012, the |
| 13 | | Department shall print, on its standard individual income tax |
| 14 | | form, a provision indicating that, if the taxpayer wishes to |
| 15 | | contribute to the Childhood Cancer Research Fund, as authorized |
| 16 | | by this amendatory Act of the 97th General Assembly, then he or |
| 17 | | she may do so by stating the amount of the contribution (not |
| 18 | | less than $1) on the return and indicating that the |
| 19 | | contribution will reduce the taxpayer's refund or increase the |
| 20 | | amount of payment to accompany the return. The taxpayer's |
| 21 | | failure to remit any amount of the increased payment reduces |
| 22 | | the contribution accordingly. This Section does not apply to |
| 23 | | any amended return.
