AMENDMENT TO SENATE BILL 40




2    AMENDMENT NO. ______. Amend Senate Bill 40 on page 13, line 
33, after "shall", by inserting ", upon request,"; and


 
4on page 13, line 7, after "promptly", by inserting ", within 10 
5days before the hearing,"; and
 
6on page 17, line 8, after "minimum", by inserting ", which 
7shall be reviewed by the Board within 120 days"; and
 
8on page 17, line 9, after "service", by inserting "within an 
9existing healthcare facility"; and
 
10on page 17, line 10, after "service", by inserting "within an 
11existing healthcare facility"; and
 
12on page 23, by replacing lines 5 through 7 with the following: 
13"holder submitting the required information and reports.

1Beginning 6 months after the effective date of this amendatory 
2Act of the 97th General Assembly, the Board shall notify, in 
3writing, a permit holder of the due date for the post-permit 
4requirements no later than 30 days before the due date for the
