| 3 | | on page 2, by replacing lines 24 through 26 with the following: |

cases. Claims and encounter data submitted under this |

shall not include a patient's name,

| 6 | | Security number; provided, however, that the Department may |
| 7 | | require, by rule, the inclusion of a unique patient identifier |
| 8 | | that may be based upon the last four digits of the patient's |
| 9 | | Social Security number. The Department shall promulgate |
| 10 | | regulations to protect the patient's rights of confidentiality |
| 11 | | and privacy. The regulations shall ensure that patient names, |
| 12 | | addresses, social security numbers, or any other data that the |
| 13 | | Department believes could be used to determine the identity of |
| 14 | | an individual patient shall be stored and processed in the most |
| 15 | | secure manner possible.".
