| 1 | | AMENDMENT TO SENATE BILL 1877

| 2 | | AMENDMENT NO. ______. Amend Senate Bill 1877 on page 17, by |
| 3 | | replacing lines 17 through 24 with the following: |
| 4 | | "in writing and deliver it to my health care provider. The |
| 5 | | authority given to the person named as my agent to serve as my |
| 6 | | "personal representative" as defined under HIPAA and |
| 7 | | regulations thereunder and to access my individually |
| 8 | | identifiable health information or authorize the release of the |
| 9 | | same to third parties shall take effect immediately, even if I |
| 10 | | designate in Paragraph 3 of this document that this agency |
| 11 | | shall otherwise take effect at some future date."; and

| 12 | | on page 20, by replacing lines 10 through 13 with the |
| 13 | | following:
|
| 14 | | "Attorney Act. Your agent can act immediately, unless you |
| 15 | | specify otherwise; but you cannot specify otherwise with |
| 16 | | respect to your "personal representative" under subparagraph |
| 17 | | D(iii).)".
