| 2 | | AMENDMENT NO. ______. Amend Senate Bill 3441 as follows:

| 3 | | on page 26, immediately below line 1, by inserting the |
| 4 | | following: |
| 5 | | "An employer who is eligible to participate in the State's |
| 6 | | medical assistance program may be designated as an alternate |
| 7 | | payee by an individual practitioner who is employed by the |
| 8 | | payee. The employer who qualifies as a payee for more than 4 |
| 9 | | practitioners and the employer who is not licensed in the same |
| 10 | | profession as the practitioner in his or her employ who has |
| 11 | | designated the employer as an alternate payee shall be subject |
| 12 | | to enhanced screening and verification by the Department. A |
| 13 | | corporation may be named as an alternate payee. A corporation |
| 14 | | registered with the Secretary of State to do business in the |
| 15 | | State of Illinois and whose shares of ownership are not |
| 16 | | publicly traded in a recognized stock exchange within the |
| 17 | | United States of America shall be subject to enhanced screening |
