AMENDMENT TO HOUSE BILL 2760 




2    AMENDMENT NO. ______. Amend House Bill 2760 on page 2, by 
3replacing lines 2 through 9 with the following:
4    "(2)  The transition period may not exceed 90 days of 
5skilled home care services except as provided by paragraph (3) 
6of this Section.
7    (3)  The transition period may be up to 180 days of skilled 
8home care services for patients where home health services are 
9needed to care for 2 or more medical conditions requiring 
10intensive management by 2 or more physicians, with 
11documentation supporting unsuccessful efforts in identifying 
12an appropriate physician in Illinois within the initial 90-day 
13time period.
14    (4)  Any significant change in the patient's condition shall 
15be communicated by the Illinois licensed agency to the 
16out-of-state physician and the patient's Illinois licensed 
17physician.
18    (5)  A copy of the license or evidence of licensure by
