| 2 | | AMENDMENT NO. ______. Amend House Bill 3190 as follows:

| 3 | | on page 7, line 2, after the period, by inserting "Proof of |
| 4 | | having received an immunization containing meningococcal |
| 5 | | conjugate vaccine shall consist of documentation of one dose of |
| 6 | | the MCV4 vaccine for 6th grade entrance and documentation of 2 |
| 7 | | doses for 12th grade entrance, unless the first dose was |
| 8 | | administered to a child who was 16 years of age or older, in |
| 9 | | which case documentation of only one dose is required at 12th |
| 10 | | grade entrance."; and

| 11 | | on page 12, line 14, after "Practices.", by inserting "The |
| 12 | | immunization shall consist of one dose of the MCV4 vaccine for |
| 13 | | 6th grade entrance and 2 doses for 12th grade entrance, unless |
| 14 | | the first dose was administered to a child who was 16 years of |
| 15 | | age or older, in which case only one dose is required at 12th |
