| 2 | WHEREAS, The members of the Illinois House of | ||||||
| 3 | Representatives are pleased to congratulate Richard F. Kotz of | ||||||
| 4 | Glencoe on his retirement from the Illinois Comprehensive | ||||||
| 5 | Health Insurance Plan (CHIP) Board; and
| ||||||
| 6 | WHEREAS, Richard Kotz was appointed to the Board on July 1, | ||||||
| 7 | 1987 and served until October 26, 2012; he is one of the | ||||||
| 8 | original and longest serving members of the Board; he served as | ||||||
| 9 | CHIP Board Secretary and was a member of the Finance Committee | ||||||
| 10 | and Evaluation Committee; and
| ||||||
| 11 | WHEREAS, Richard Kotz works as a consultant and is a | ||||||
| 12 | retired attorney; and
| ||||||
| 13 | WHEREAS, Richard Kotz is a former Vice-President - Law and | ||||||
| 14 | Deputy General Counsel of Sears, Roebuck, and Company; he is a | ||||||
| 15 | former member of the Illinois Assembly on the Uninsured, former | ||||||
| 16 | President and Executive Committee member and current board | ||||||
| 17 | member of Mental Health America in Illinois, and current | ||||||
| 18 | Treasurer and board member of Health and Disability Advocates; | ||||||
| 19 | he is an Advisory Board Member of the Psychology Doctorate | ||||||
| 20 | Program at the Chicago School of Professional Psychology and is | ||||||
| 21 | a former consultant to the National Institute of Mental Health; | ||||||
| 22 | he is a former Glencoe Village Trustee, former President and | ||||||
| |||||||
| |||||||
| 1 | board member of the Midwest Regional Group, former National | ||||||
| 2 | Board Member of the American Society of Corporate Secretaries, | ||||||
| 3 | and former member of the American and Chicago Bar Associations; | ||||||
| 4 | he served as Chairman of the Securities Law Committee of the | ||||||
| 5 | Chicago Bar Association; and
| ||||||
| 6 | WHEREAS, Richard Kotz has been married to Debbie for 48 | ||||||
| 7 | years and they have 3 children and several grandchildren; he is | ||||||
| 8 | a graduate of the Wharton School of Finance and Commerce of the | ||||||
| 9 | University of Pennsylvania and the University of Pennsylvania | ||||||
| 10 | School of Law; he has a Masters of Business Arts from The | ||||||
| 11 | American University; he is an avid reader of history novels and | ||||||
| 12 | mystery books; he belongs to a history club and enjoys theatre, | ||||||
| 13 | music, and traveling; therefore, be it
| ||||||
| 14 | RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE | ||||||
| 15 | NINETY-EIGHTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that | ||||||
| 16 | we congratulate Richard F. Kotz on his retirement from the | ||||||
| 17 | Illinois Comprehensive Health Insurance Plan Board and wish him | ||||||
| 18 | the best in all his future endeavors; and be it further
| ||||||
| 19 | RESOLVED, That a suitable copy of this resolution be | ||||||
| 20 | presented to Richard Kotz as an expression of our esteem and | ||||||
| 21 | respect.
