| 4 | | with a permit provided under Section 8.1 of the Illinois Dental |

| 6 | | on page 2, line 5, by replacing "26" with "19"; and

| 7 | | on page 2, by replacing line 8 with the following:

| 9 | | visits to the dental care provider prior to accessing other |
| 10 | | coverage under this subsection.".
