| 2 | | Be it enacted by the People of the State of Illinois, |
| 3 | | represented in the General Assembly:

| 4 | | Section 5. The College Student Immunization Act is amended |
| 5 | | by changing Section 6 as follows:
|

| 7 | | Sec. 6. Report. Within 8 weeks after the

instruction, the post-secondary educational |
| 9 | | institution shall file a report
with the Department stating the |
| 10 | | number of persons attending the institution
who had presented: |
| 11 | | proof of immunization; certification of medical
exemption; |
| 12 | | statement of religious objection; and no proof of immunization.
