AMENDMENT TO HOUSE BILL 1514




2    AMENDMENT NO. ______. Amend House Bill 1514 by replacing 
3everything after the enacting clause with the following:


 
 
4    "Section 5. The Illinois Public Aid Code is amended  by 
5adding Section 12-4.49 as follows:
 
6    (305 ILCS 5/12-4.49 new)
7    Sec. 12-4.49. Diabetes Prevention Pilot Program. The 
8Department of Healthcare and Family Services shall create a 
93-year Diabetes Prevention Pilot Program to establish an 
10evidence-based diabetes prevention program in St. Clair 
11County, DeKalb County, and Rock Island County. To implement the 
12pilot program, the Department shall designate a single 
13community-based, not-for-profit provider of healthy living 
14services to manage and administer the pilot program. The 
15Department shall require the designated community-based, 
16not-for-profit provider to contract with a managed care plan to

1provide the diabetes services offered under the pilot program. 
2The Department shall adopt rules to implement this Section.
 
 
3    Section 99. Effective date. This Act takes effect upon 
4becoming law.".
