AMENDMENT TO HOUSE BILL 4249




2    AMENDMENT NO. ______. Amend House Bill 4249 by replacing 
3everything after the enacting clause with the following:

 
 
4    "Section 5. The Nursing Home Care Act is amended  by 
5changing Section 2-106a as follows:
 



6    (210 ILCS 45/2-106a)



7    Sec. 2-106a. Resident identification wristlet. An 
8identification wristlet may be employed for any resident upon a 
9physician's order, which shall document the need for the 
10identification wristlet in the resident's clinical record.  A 
11facility may require all residents residing in an Alzheimer's 
12disease unit to wear an identification wristlet as a condition 
13of admission. All identification wristlets shall include, at a 
14minimum, No identification
wristlets shall be employed except 
15as ordered by a physician who documents the
need for such 
16mandatory identification in the resident's clinical record.

identification bracelets are required, they must identify 
2the resident's name,
and the name, telephone number, and 
3address of the facility issuing the identification wristlet.
