1 AMENDMENT TO HOUSE BILL 5775

2 AMENDMENT NO. ______. Amend House Bill 5775 on page 4, by

3 replacing lines 7 and 8 with the following:

4 "witnessed voluntary acknowledgment of paternity ~~parentage~~ and

5 denial of parentage ~~paternity~~ to the Department of Healthcare

6 and Family"; and

7 on page 8, immediately below line 5, by inserting the

8 following:

9 "(10) In the event new data elements are included in the

10 voluntary acknowledgment of paternity form, denial of

11 parentage form, or rescission of voluntary acknowledgment of

12 paternity or denial of parentage form, the Department of

13 Healthcare and Family Services, in conjunction with the

14 Department of Public Health, shall provide instructions about

15 the new data elements to the hospital personnel responsible for

16 assisting the child's mother, biological father, or presumed

17 father with completing the forms.".
