| 2 | | WHEREAS, The members of the Illinois House of |
| 3 | | Representatives are saddened to learn of the death of Michael |
| 4 | | Frank Chornak of Marion, who passed away on August 2, 2015; and
|
| 5 | | WHEREAS, Michael Chornak was born in Mount Vernon on August |
| 6 | | 19, 1958 to Frank and Mollie (nee Hogg) Chornak; he graduated |
| 7 | | from Marion High School in 1976, where he was a well-known pole |
| 8 | | vaulter; he received his degree in education, and later, his |
| 9 | | master's in administration from Southern Illinois University |
| 10 | | Carbondale; he married Kellie Winemiller; and
|
| 11 | | WHEREAS, Michael Chornak started his career teaching and |
| 12 | | coaching in Carterville; he then joined Marion High School as a |
| 13 | | health instructor and coach; he became the Marion athletic |
| 14 | | director in 1995; and
|
| 15 | | WHEREAS, Michael Chornak was a member of Second Baptist |
| 16 | | Church in Marion and also enjoyed attending First Baptist |
| 17 | | Church in Marion; his other memberships included the Southern |
| 18 | | Illinois Coaches Association and the Illinois Athletic |
| 19 | | Directors Association; he was elected the 2012-2013 Class 3A |
| 20 | | and 4A Division Athletic Director of the Year; he was a board |
| 21 | | member for the Marion Country Club and a member of the |
| 22 | | Fellowship of Christian Athletes; he started and sponsored the |

| 1 | | first bass fishing team at Marion High School; and
|
| 2 | | WHEREAS, Michael Chornak was an avid hunter and fisherman |
| 3 | | in his free time; he loved fishing in all of the lakes in |
| 4 | | Southern Illinois, but especially enjoyed traveling to Canada |
| 5 | | and Alabama to fish; he also enjoyed hunting deer at Wade |
| 6 | | Hudgens' farm and goose hunting at Pike's Hunting Club; he |
| 7 | | loved his friends and family and all of the fun they had |
| 8 | | camping, cooking out, going to St. Louis Cardinal baseball |
| 9 | | games and Holiday World, and just "hanging out"; and
|
| 10 | | WHEREAS, Michael Chornak was preceded in death by his |
| 11 | | parents, and aunts and uncles; and
|
| 12 | | WHEREAS, Michael Chornak is survived by his son, Collin; |
| 13 | | his daughter, Sydney; his sister, Kathy Dallas and her husband, |
| 14 | | Mark; his niece, Marcy Dallas; his nephew, Brax Dallas; his |
| 15 | | aunt, Clarebel Gray; and many cousins; therefore, be it
|
| 16 | | RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE |
| 17 | | NINETY-NINTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we |
| 18 | | mourn the passing of Michael Frank Chornak, and extend our |
| 19 | | sincere condolences to his family, friends, and all who knew |
| 20 | | and loved him; and be it further
