| 2 | | WHEREAS, The members of the Illinois House of |
| 3 | | Representatives are saddened to learn of the death of Reginald |
| 4 | | D. "Hats" Adams of Calumet Park; and
|
| 5 | | WHEREAS, Reginald Adams has been the Director of Community |
| 6 | | Affairs at Rush University Medical Center since 1974; prior to |
| 7 | | that he served as Community Liaison and Employee Relations |
| 8 | | Coordinator; he joined the Rush staff in 1968, when he was |
| 9 | | hired as Chief Youth Worker at the Mile Square Health Care |
| 10 | | Center; before working at Rush, he worked for the Chicago Boys |
| 11 | | and Girls Clubs; and
|
| 12 | | WHEREAS, Reginald Adams created S.A.M.E. (Science and Math |
| 13 | | Excellence Network) in 1990 and most recently served as |
| 14 | | President of its Board of Directors; S.A.M.E. is a member of |
| 15 | | the Partner for Education, a
consortium of science-based |
| 16 | | institutions, construction companies, and
community-based |
| 17 | | organizations that collaborate on assisting the
community |
| 18 | | organizations with their science and math programs; Rush and |
| 19 | | its corporate partners sponsor after-school science clubs,
|
| 20 | | provide judging at local science fairs, offer summer training |
| 21 | | programs for teachers and
sponsor a mobile science lab that |
| 22 | | visits schools without laboratory facilities; in fiscal year |
| 23 | | 2010, 3,398 children and teens benefited from S.A.M.E.'s |
|

| 2 | | WHEREAS, Reginald Adams received numerous awards and |
| 3 | | honors throughout his career, including the Rush University |
| 4 | | Medical Center Patient Access Committee Community Service |
| 5 | | Award 2010, 2009, 2008; the Dr. Martin Luther King, Jr. Life |
| 6 | | Achievement Award, 2008; the Rush University Medical Center |
| 7 | | Trustee Medal, 2007; the Feld Entertainment |
| 8 | | Circus-in-a-Classroom Award, 2006; the Illinois |
| 9 | | Business-Education Exemplary Partnership Award, 2002; the |
| 10 | | Lawndale Community Service Award, 2001; the Chicago Area |
| 11 | | Project Certificate of Appreciation Award, 2000; the Henry |
| 12 | | Horner Alumni Association Health and Education Award, 1997; the |
| 13 | | Westside Association for Community Action (WACA) Service |
| 14 | | Award, 1996; the Dr. Martin Luther King, Jr. Humanitarian |
| 15 | | Award, 1996; the YMCA Westside Association for Community Action |
| 16 | | Humanitarian Award, 1996; the YMCA Southside Chairman's |
| 17 | | Roundtable Member Community Service Award, 1996; the Malcom X |

| 3 | | daughter, Regina Adams; and

| 4 | | WHEREAS, Reginald Adams is survived by his wife, Constance |
| 5 | | Adams; his children, Reginald L. Adams and Janice Adams; his 3 |
| 6 | | sisters, Doris McKinnie, Leontine Jordan, and Arlene |
| 7 | | Bradley-Walton; his 2 brothers, Randolph Adams and Derrell |
| 8 | | Adams; his 6 grandchildren; and his 5 great-grandchildren; |
| 9 | | therefore, be it

| 10 | | RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE |
| 11 | | NINETY-NINTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we |
| 12 | | mourn the passing of Reginald D. "Hats" Adams, and extend our |
| 13 | | sincere condolences to his family, friends, and all who knew |
| 14 | | and loved him; and be it further

| 15 | | RESOLVED, That a suitable copy of this resolution be |
| 16 | | presented to the family of Reginald Adams as an expression of |
| 17 | | our deepest sympathy.
