Amends the Department of Public Health Act. Requires the Department of Public Health to establish a 3-year pilot program that provides diabetes testing and education on diabetes management in Cook County, DeKalb County, and Rock Island County from funds appropriated for that purpose. Provides that the Department shall designate a single community-based, not-for-profit provider of healthy living services to manage and administer the program; and that the designated provider shall receive reimbursement under the medical assistance program for the diabetes testing and educational services provided under the pilot program. Grants the Department rulemaking authority. Effective immediately.

2 | | Be it enacted by the People of the State of Illinois,

3 | | represented in the General Assembly:

4 | | Section 5. The Department of Public Health Act is amended |
5 | | by adding Section 8.5 as follows: |
6 | | (20 ILCS 2305/8.5 new) |
7 | | Sec. 8.5. Diabetes testing and diabetes management; pilot |
8 | | program. From funds appropriated for that purpose, the |
9 | | Department of Public Health shall establish a 3-year pilot |
10 | | program that provides diabetes testing and education on |
11 | | diabetes management in Cook County, DeKalb County, and Rock |
12 | | Island County. To implement the pilot program, the Department |
13 | | shall designate a single community-based, not-for-profit |
14 | | provider of healthy living services to manage and administer |
15 | | the program. The designated provider shall receive |
16 | | reimbursement under the medical assistance program for the |
17 | | diabetes testing and educational services provided under the |
18 | | pilot program. The Department shall adopt rules in cooperation |
19 | | with the Department of Healthcare and Family Services to |
20 | | implement this Section.

21 | | Section 99. Effective date. This Act takes effect upon |
22 | | becoming law.
