Creates the Patient Intake Form Military Veteran Inquiry Act. Provides that a health care provider must include on each patient intake form a question asking whether the patient or their spouse has ever served in the military for purposes of tailoring treatment options. Defines "health care provider" and "military veteran".

| 2 | | Be it enacted by the People of the State of Illinois,
|
| 3 | | represented in the General Assembly:
|
| 4 | | Section 1. Short title. This Act may be cited as the |
| 5 | | Patient Intake Form Military Veteran Inquiry Act.
|
| 6 | | Section 5. Purpose. The purpose of this Act is to improve |
| 7 | | the tailoring of treatment options for military veterans.
|
| 8 | | Section 10. Definition. As used in this Act: |
| 9 | | "Health care provider" means any physician or public or |
| 10 | | private facility that provides, on an inpatient or outpatient |
| 11 | | basis, preventive, diagnostic, therapeutic, convalescent, |
| 12 | | rehabilitation, mental health, or intellectual disability |
| 13 | | services, including general or special hospitals, skilled |
| 14 | | nursing homes, extended care facilities, intermediate care |
| 15 | | facilities, and mental health centers. |
| 16 | | "Military veteran" means a person who has served in any |
| 17 | | component of the U.S. Armed Forces, including reservists; |
| 18 | | members of the National Guard of the United States; or the |
| 19 | | National Guard of any state, the District of Columbia, a |
| 20 | | commonwealth, or a territory of the United States.
