| 4 | | on page 3, lines 15 and 16, by deleting "be it further"; and
|
| 5 | | on page 3, immediately below line 16, by inserting the |
| 6 | | following: |
| 7 | | "(4) For each child meeting the criteria in subsection (1), |
| 8 | | (2), or (3) the following information: who was subsequently |
| 9 | | placed, how long it took the child to be placed, and whether |
| 10 | | the child was placed consistent with clinical recommendations; |
| 11 | | and be it further".
