SENATE RESOLUTION

  
 
2    WHEREAS, The members of the Illinois Senate are saddened to 
3learn of the death of Phyllis Fondakowski of Woodstock, who 
4passed away on February 12, 2015; and
  
 
5    WHEREAS, Phyllis Fondakowski was born in Chicago on May 1, 
61915 to Frank and Angelina (nee Scala) Marolda; she married 
7Raymond Fondakowski in Chicago on January 12, 1946; and
  
 

  
8    WHEREAS, Phyllis Fondakowski was preceded in death by her 
9husband, Raymond; her daughter, Susan Farrell; her parents; and 
10her siblings, John, Mary, Phillip, Gerard, and Lucille; and
  
 
11    WHEREAS, Phyllis Fondakowski is survived by her 2 
12daughters, Pamela (William) Fritz and Denise (Stephen) Scalzo; 
13her 4 grandchildren, Erich (Jean) Fritz, Marc Fritz, Matthew 
14(Marcie) Scalzo, and Nicholas Scalzo; her great-granddaughter, 
15Samantha Scalzo; and her brother, Dan Marolda; therefore, be it
  
 
16    RESOLVED, BY THE SENATE OF THE NINETY-NINTH GENERAL 
17ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of 
18Phyllis Fondakowski, and extend our sincere condolences to her 
19family, her friends, and  all who knew and loved her; and be it
