WHEREAS, The members of the Illinois House of Representatives are
pleased to honor milestones in the lives of the citizens of the State
of Illinois; and
WHEREAS, It has come to our attention that Frank J. Wsol is
retiring this year after loyally serving the Teamster's Union for over
fifty years; and
WHEREAS, Frank J. Wsol served his country in the United States Air
Force in Japan and Korea in the mid-1940's; and
WHEREAS, Frank J. Wsol has served with the Teamsters since 1946,
when he became a member of Local No. 761; he became a member of I.B.T.
Local No. 710 while driving a meat delivery truck in 1951; he was
appointed organizer in 1966 by John T. (Sandy) O'Brien; he was promoted
to business agent in 1971 by William D. Joyce; and
WHEREAS, In 1981 Mr. Wsol was appointed Vice President; in 1992 he
was nominated President of Local No. 710; in 1993 he was appointed
Secretary-Treasurer; Mr. Wsol has served as trustee of the Local No.
710 Health, Welfare, and Pension funds and became chairman of those
funds in June of 1993; and
WHEREAS, Frank Wsol has served as chairman of the Illinois Joint
State Grievance Committee, the Meat, MCLAC, and Yellow Freight
Grievance Committee, and the I.B.T. National Grievance Committee; he is
a member of the All Teamster Neer-Goudie American Legion Post No. 846;
he also serves on the Board of Directors for Children's Advocacy Center
and the Little City Foundation in Palatine, Illinois; and
WHEREAS, Frank Wsol and his wife, Florence, will spend their free
time with their sons and grandchildren; therefore, be it
RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-SECOND
GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we congratulate Frank
J. Wsol for his dedication and service to the Teamster's Union for over
fifty years, and we wish him well in all of his future endeavors; and

HOUSE RESOLUTION 38

WHEREAS, President Ronald Wilson Reagan, a man of humble
background, worked throughout his life serving freedom and advancing
the public good, having been employed as an entertainer, Union leader,
corporate spokesman, Governor of California and President of the United
States; and
WHEREAS, Ronald Reagan served with honor and distinction for two
terms as the 40th President of the United States of America; the second
of which he was victorious in 49 of the 50 states in the general
election, earning the confidence of 3/5 of the electorate, a record
unsurpassed in the history of American presidential elections; and
WHEREAS, in 1981, when Ronald Reagan was inaugurated President, he
inherited a disillusioned nation shackled by rampant inflation and high
unemployment; and
WHEREAS, During Mr. Reagan's presidency he worked in a bipartisan
manner to enact his bold agenda of restoring accountability and common
sense to Government which led to an unprecedented economic expansion
and opportunity for millions of Americans; and
WHEREAS, Mr. Reagan's commitment to an active social policy agenda
for the nation's children helped lower crime and drug use in our
neighborhoods; and
WHEREAS, President Reagan's commitment to our armed forces
contributed to the restoration of pride in America, her values and
those cherished by the free world, and prepared America's Armed Forces
to win the Gulf War; and
WHEREAS, President Reagan's vision of "peace through strength" led
to the end of the Cold War and the ultimate demise of the Soviet Union,
guaranteeing basic human rights for millions of people; and
WHEREAS, on February 6, 2001 Ronald Reagan will have reached the
age of ninety years, thus becoming both the oldest active and oldest

former President in the history of our nation; therefore, be it

HOUSE RESOLUTION 4
Offered by Representatives Black - Garrett:
WHEREAS, Since Social Security numbers were introduced in 1934 to
track employee earnings records, the nine-digit code has almost become
a national identification number; and
WHEREAS, Numerous government agencies, organizations, and
businesses, from Girl Scout troops to credit agencies, require or
request Social Security numbers; and
WHEREAS, Social Security numbers are widely used as the key to a
storehouse of personal and private information; and
WHEREAS, Identity theft is the fastest growing crime in the nation;
in 1999, allegations of identity theft using Social Security numbers
more than doubled, from 26,531 to 62,000 cases; therefore, be it
RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-SECOND
GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we urge the Attorney
General of the State of Illinois to review the practice of requiring or
requesting the disclosure of Social Security numbers to apply for or
renew a license or permit issued by the State; and be it further
RESOLVED, That a suitable copy of this resolution be delivered to
the Attorney General.
HOUSE RESOLUTION 7
Offered by Representative Wirsing:
WHEREAS, Congestive heart failure is a major health problem that
impacts five million Americans, or 1.5% of the United States
population; and
WHEREAS, Each year there 550,000 new cases of congestive heart
failure diagnosed, with 200,000 deaths and one million hospitalizations
per year due to the disease; and
WHEREAS, Congestive heart failure is the leading cause of
hospitalization in patients over the age of 65; and
WHEREAS, Congestive heart failure patients with the most severe
symptoms have approximately 50% one-year survival rates and half of
patients with even mild disease die within 5 years; and
WHEREAS, The vast majority of patients are not diagnosed until late
in the course of the disease; and
WHEREAS, A conservative estimate of annual U.S. health expenditures
for congestive heart failure is in excess of ten billion dollars, with
the largest portion, eight billion dollars of this expenditure, for the
cost of hospitalizations; and
WHEREAS, Although current treatment guidelines recommend that
approximately 80% of heart failure patients receive ACE inhibitors in
combination with beta blockers to extend life and reduce costly
hospitalizations, but only 45% of patients are treated with ACE
inhibitors and 20% of patients receive beta blockers; and
WHEREAS, The primary goals of "National Heart Failure Awareness
Week" are (1) to promote research related to all aspects of heart
failure and provide a forum for presentation of that research; (2) to
educate heart failure caregivers and patients through programs,
publications, and other media allowing for more effective treatment and
diagnosis of heart failure; and (3) to enhance the quality and duration

of life for those with heart failure; therefore, be it
RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-FIRST
GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we endorse the national
proclamation of National Heart Failure Awareness Week; and be it
further
RESOLVED, That the Secretary of the Department of Public Health
transmit the treatment guidelines for congestive heart failure,
published by the Heart Failure Society of America, Inc., to the
appropriate health care professionals in the State of Illinois, while
urging their adoption of these guidelines in their practice; and be it
further
RESOLVED, That physicians and payers should be encouraged to
prescribe or provide reimbursement for the standard of care for
congestive failure, including FDA approved beta blockers and ACE
inhibitors as well as other therapies that have been shown to reduce
the mortality and morbidity associated with congestive heart failure;
and be it further
RESOLVED, That the Illinois General Assembly endorses the inclusion
of the latest treatment advances for congestive heart failure,
including ACE inhibitor and beta blocker therapies in the curriculum of
medical schools in the State of Illinois.
HOUSE RESOLUTION 32
Offered by Representatives Lou Jones - Feigenholtz:
WHEREAS, The first Illinois families will soon be reaching their
60-month TANF lifetime eligibility limitation for public assistance
grants and supportive services; and
WHEREAS, The Illinois Public Aid Code at 305 ILCS 5/4-1.12 permits
the Department of Human Services to exempt individual assistance units
from this 60-month limitation; and
WHEREAS, The Department has not yet established how it will resolve
addressing the many families that reach their 60-month lifetime
limitation under the TANF program; therefore, be it
RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-SECOND
GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that the Department of Human
Services shall conduct full case reviews 6 months before each family
reaches its 60-month TANF limitation to determine, by the end of the
60-month limitation period, what can be done to overcome the need for
future public assistance for the family; and be it further
RESOLVED, That the Department of Human Services shall report to the
House of Representatives, by March 30, 2001, the manner in which it
intends to ensure that impoverished families will be housed, fed, and
clothed, have their necessary medical care covered, and be otherwise
protected once their 60-month TANF eligibility has expired; and be it
further
RESOLVED, That the Department of Human Services shall include in
its report an estimate of how much State funding would be necessary to
cover the costs of a TANF continuation program funded with only State
moneys for as many of these families as the Department recommends be
continued in a TANF-like program, with eligibility for such a
continuation program renewable in 3-month or 6-month increments for
each family, based on the family's hardship circumstances and good
faith efforts to become self-supporting and the family's continued need
for assistance; and be it further
RESOLVED, That the Department of Human Services shall also include
in its report the Department's recommended statutory changes that may
be necessary to implement such a TANF continuation program for Illinois
families, and the recommended manner of amending the Department's
appropriations line items to accommodate such a program.

WHEREAS, Throughout history brave Americans have shed their blood

thousands have paid the ultimate sacrifice to ensure that future
generations enjoy life's liberties; and
WHEREAS, On August 7, 1782, General George Washington established
the Military Badge of Merit, which on February 22, 1932 became the
present and now the oldest military decoration in the world, the Purple
Heart medal; and
WHEREAS, The Purple Heart medal is awarded to all military
personnel who are killed or wounded in action against the enemy;
therefore, be it
RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-SECOND
GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, THE SENATE CONCURRING
HEREIN, that Interstate 72, traversing through the heart of Illinois,
be designated as the commemorative "Purple Heart Memorial Highway", to
pay tribute to the many thousands of Illinois residents who have been
awarded the Purple Heart medal; and be it further
RESOLVED, That the Illinois Department of Transportation is
requested to erect at suitable locations, consistent with State and
federal regulations, an appropriate plaque or signs giving notice of
the name; and be it further

WHEREAS, Public Act 91-930 amended the Illinois Income Tax Act to
create income tax credits for employee child care; and
WHEREAS, The Act provides for a credit of 30% of the start-up costs
expended by a corporation to provide a child care facility for the
children of its employees; this credit is available beginning with
taxable years ending on or after December 31, 2000 and ending with
taxable years ending on or before December 31, 2004; and
WHEREAS, The Act provides for an additional credit of 5% of the
annual amount paid by the corporation in providing the child care
facility for employee's children; this credit is available beginning
with taxable years ending on or after December 31, 2000 and does not
have an expiration date; and
WHEREAS, The Act provides that, to be eligible for the credit, a
corporation may provide and operate a child care facility independently
or in partnership with one or more other corporations; the Act limits
the credit to child care facilities located in Illinois; and
WHEREAS, It is in the best interest of the citizens of Illinois
that the State promote the availability of quality child care for the
taxpayers of Illinois; therefore be it
RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE NINETY-SECOND
GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, THE SENATE CONCURRING
HEREIN, that the Department of Commerce and Community Affairs, in
cooperation with the Department of Revenue, shall track and monitor the
use by Illinois employers of the tax credits for employee child care
created by Public Act 91-930; and be it further
RESOLVED, that the Department of Commerce and Community Affairs, in
cooperation with the Department of Revenue, shall report to the
Governor's Children and Family Leadership Subcabinet by March 1, 2002,
and each March 1 thereafter, on the extent the credits are used and, if
the credits are not used as expected, the reasons why employers are not
using the credits; and be it further
RESOLVED, that suitable copies of this resolution be delivered to
the Director of Commerce and Community Affairs, the Director of
Revenue, and the Governor.

WHEREAS, The purpose of the forums was to hear firsthand from local
leaders about the challenges and opportunities facing rural families
and businesses; and
WHEREAS, The Rural Affairs Council's report, "Charting a Course for
the New Millennium," details the issues raised at the forums; and
WHEREAS, The Rural Affairs Council is committed to developing and
implementing action strategies for rural growth and prosperity; and
WHEREAS, The report provides a foundation for coordinated actions
and public-private partnerships to encourage rural growth and
prosperity; and
WHEREAS, The Rural Affairs Council hopes the General Assembly and
the Governor will continue to work toward strengthening the rural
economy, providing needed services to rural areas, and improving the
quality of life for rural residents; therefore, be it
