 1                    AMENDMENT TO HOUSE BILL 3486

 2        AMENDMENT NO.     .  Amend House Bill  3486  on  page  2,
 3    line 23 by inserting "judicial" after "any"; and

 4    on  page  2, line 29 by changing "reasonable" to "at least 48
 5    hours'"; and

 6    on page  2,  line  31  by  inserting  after  the  period  the
 7    following:

 8    "The  employer  may require certification within a reasonable
 9    time of the need for leave under this Act from the  employee.
10    Certification  is  sufficient  in  the  form  of  any  of the
11    following:
12        (A)  A police report indicating that the employee  was  a
13    victim of domestic violence.
14        (B)  A  court order protecting or separating the employee
15    from the perpetrator of an act of domestic violence, or other
16    evidence from the court  or  prosecuting  attorney  that  the
17    employee has appeared in court.
18        (C)  Documentation  from a medical professional, domestic
19    violence advocate, health care provider,  or  counselor  that
20    the  employee was undergoing treatment for physical or mental
21    injuries or abuse resulting in victimization from an  act  of
22    domestic violence."; and

 2    occurs" to "If advance notice is not feasible"; and

 3    on page 3, line 22 by inserting  "solely"  after  "employer";
 4    and

 5    on  page  3,  line 34 by inserting "solely" after "employer";
 6    and

 7    on page 5, line 1 by changing "reasonable" to  "at  least  48
 8    hours'"; and

 9    on  page  5,  line 26 by inserting "solely" after "employer";
10    and

11    on page 6, line 4 by inserting "solely" after "employer".
