2     AMENDMENT NO. ______. Amend House Bill 4924 on page 1, by
3 replacing line 11 through line 14 with the following:
4 "96th General Assembly must provide coverage for an operation,
5 either monaurally or binaurally, to implant cochlear implants
6 and cochlear devices, including all internal and external
7 components, and provide post-treatment services, including,
8 but not limited to, programming, troubleshooting, repairs,
9 replacement components, such as external speech processors,
10 microphones, coils, headsets, cables, and batteries, FM
11 systems, auditory training, aural rehabilitation, and speech
12 therapy for children identified by 18 years of age as having
13 hearing impairment to the degree that they would benefit from
14 implantation. These services must be provided by a speech
15 pathologist, audiologist, or physician licensed to practice in
16 this State.".
