1     AN ACT concerning State government.
 
2     Be it enacted by the People of the State of Illinois,
3 represented in the General Assembly:
 
4     Section 5. The Illinois Identification Card Act is amended
5 by changing Section 12 as follows:
 
6     (15 ILCS 335/12)  (from Ch. 124, par. 32)

8     Sec. 12. Fees concerning Standard Illinois Identification
9 Cards. The fees required under this Act for standard Illinois
10 Identification Cards must accompany any application provided
11 for in this Act, and the Secretary shall collect such fees as
12 follows:
13    a. Original card issued on or before
14        December 31, 2004.......................... $4
15        Original card issued on or after
16        January 1, 2005............................ $20
17    b. Renewal card issued on or before
18        December 31, 2004...........................4
19        Renewal card issued on or after
20        January 1, 2005............................ 20

21     All fees collected under this Act shall be paid into the
22 Road Fund of the State treasury, except that the following
23 amounts shall be paid into the General Revenue Fund: (i) 80% of
24 the fee for an original, renewal, or duplicate Illinois
25 Identification Card issued on or after January 1, 2005; and
26 (ii) 80% of the fee for a corrected Illinois Identification

1 Card issued on or after January 1, 2005.
2     Any disabled person making an application for a standard
3 Illinois Identification Card for no fee must, along with the
4 application, submit an affirmation by the applicant on a form
5 to be provided by the Secretary of State, attesting that such
6 person is a disabled person as defined in Section 4A of this
7 Act.
8     An individual, who resides in a veterans home or veterans
9 hospital operated by the state or federal government, who makes
10 an application for an Illinois Identification Card to be issued
11 at no fee, must submit, along with the application, an
12 affirmation by the applicant on a form provided by the
13 Secretary of State, that such person resides in a veterans home
14 or veterans hospital operated by the state or federal
15 government.
16     If an indigent military veteran makes a written request or
17 application for an identification card and that veteran
18 provides documentation certified by a County Veterans
19 Assistance Commission verifying his or her status as an
20 indigent veteran, then the indigent military veteran shall not
21 be charged any fee for such request or application. The
22 Secretary of State, in consultation with County Veterans
23 Assistance Commissions, shall develop a form appropriate for
24 the verification of a person's status as both a veteran and an
25 indigent person and shall develop rules to implement the
26 provisions of this Section.

12 by an affirmation by a qualified person, as defined in Section
13 4C of this Act, on a form provided by the Secretary of State,
14 that the applicant is currently homeless as defined in Section
15 1A of this Act.
16     If an indigent military veteran makes a written request or
17 application for an identification card and that veteran
18 provides documentation certified by a County Veterans
19 Assistance Commission verifying his or her status as an
20 indigent veteran, then the indigent military veteran shall not
21 be charged any fee for such request or application. The
