AMENDMENT TO HOUSE BILL 5424

2     AMENDMENT NO. ______. Amend House Bill 5424 by replacing
3 everything after the enacting clause with the following:
 
4     "Section 5. The Illinois Identification Card Act is amended
5 by changing Section 12 as follows:
 
6     (15 ILCS 335/12)  (from Ch. 124, par. 32)

8     Sec. 12. Fees concerning Standard Illinois Identification
9 Cards. The fees required under this Act for standard Illinois
10 Identification Cards must accompany any application provided
11 for in this Act, and the Secretary shall collect such fees as
12 follows:
13    a. Original card issued on or before
14        December 31, 2004.......................... $4
15        Original card issued on or after
16        January 1, 2005............................ $20

1     All fees collected under this Act shall be paid into the
2 Road Fund of the State treasury, except that the following
3 amounts shall be paid into the General Revenue Fund: (i) 80% of
4 the fee for an original, renewal, or duplicate Illinois
5 Identification Card issued on or after January 1, 2005; and
6 (ii) 80% of the fee for a corrected Illinois Identification
7 Card issued on or after January 1, 2005.
8     Any disabled person making an application for a standard
9 Illinois Identification Card for no fee must, along with the
10 application, submit an affirmation by the applicant on a form
11 to be provided by the Secretary of State, attesting that such
12 person is a disabled person as defined in Section 4A of this
13 Act.
14     An individual, who resides in a veterans home or veterans
15 hospital operated by the state or federal government, who makes
16 an application for an Illinois Identification Card to be issued
17 at no fee, must submit, along with the application, an
18 affirmation by the applicant on a form provided by the
19 Secretary of State, that such person resides in a veterans home
20 or veterans hospital operated by the state or federal
21 government.
22     If an indigent military veteran makes a written request or
23 application for an identification card and that veteran
24 provides documentation certified by a County Veterans
25 Assistance Commission verifying his or her status as an
26 indigent veteran, then the indigent military veteran shall not

1 be charged any fee for such request or application. The
2 Secretary of State, in consultation with County Veterans
3 Assistance Commissions, shall develop a form appropriate for
4 the verification of a person's status as both a veteran and an
5 indigent person and shall develop rules to implement the
6 provisions of this Section.

16 Identification Card to be issued at no fee must be accompanied
17 by an affirmation by a qualified person, as defined in Section
18 4C of this Act, on a form provided by the Secretary of State,
19 that the applicant is currently homeless as defined in Section
20 1A of this Act.
21     If an indigent military veteran makes a written request or
22 application for an identification card and that veteran
23 provides documentation certified by a County Veterans
24 Assistance Commission verifying his or her status as an
25 indigent veteran, then the indigent military veteran shall not
26 be charged any fee for such request or application. The

7     Section 95. No acceleration or delay. Where this Act makes
8 changes in a statute that is represented in this Act by text
9 that is not yet or no longer in effect (for example, a Section
10 represented by multiple versions), the use of that text does
11 not accelerate or delay the taking effect of (i) the changes
12 made by this Act or (ii) provisions derived from any other
13 Public Act.".
