AMENDMENT TO HOUSE BILL 491 




2    AMENDMENT NO. ______. Amend House Bill 491 by replacing 
3everything after the enacting clause with the following:

 
4    "Section 5. The School Code is amended  by adding Section 
522-80 as follows:
 
6    (105 ILCS 5/22-80 new)
7    Sec. 22-80. Student athletes; injury incident report.
8    (a)  In this Section, "official" means a patched official as 
9defined by guidelines of the Illinois Elementary School 
10Association or the Illinois High School Association for 
11certification.
12    (b)  All officials certified by the Illinois Elementary 
13School Association or the Illinois High School Association 
14shall fill out an injury incident report following a game if an 
15athlete is unable to return to the field of play due to an 
16injury received during the game.  The report shall include

1whether the injury occurred to the upper or lower extremities 
2and a special notation for concussive or other head injuries.
3    (c)  The Illinois Elementary School Association and the 
4Illinois High School Association shall maintain a database of 
5the reports required under this Section and file the reports, 
6along with the identification and contact information of the 
7officials that filed out the reports, with the Department of 
8Public Health at the end of each respective athletic season.
 

9    Section 99. Effective date. This Act takes effect July 1, 
102013.".
