Synopsis As Introduced
Amends the Adoption Act. In provisions requiring the Department of Public Health to inform the public of the Illinois Adoption Registry and Medical Information Exchange through notices enclosed with driver's license renewal applications, deletes language providing that the notices are required only until December 31, 2010. Effective immediately.

Amends the Adoption Act. Makes various changes regarding: legislative intent; the Illinois Adoption Registry and Medical Information Exchange; the creation of the OBC-Access Public Information Campaign Oversight Committee; the Adoption Registry Advisory Council; the exchange of identifying and medical information; the release of identifying information; the Illinois Adoption Registry Application; the contents of specified forms, requests, and authorizations; confidential intermediaries; fees; and other matters. Effective immediately.

House Floor Amendment No. 2
Further amends the Adoption Act. Changes the dates for: the public information campaign; notices accompanying vehicle registration renewal applications; and filing a Request for a Non-certified Copy of an Original Birth Certificate. Changes references from "U.S. Immigration and Naturalization Service" to "U.S. Citizenship and Immigration Services". Provides for the Adoption Registry Advisory Council to collect, compile and review statistical data (instead of collecting statistical data). Provides for the filing of a death certificate or a proof of death form by a confidential intermediary or a surviving relative of a deceased birth parent (instead of the filing of a death certificate or a proof of death form). In the introductory description of various adoption forms, restores or adds language stating that the form shall be "substantially" as set forth in the statute. Provides that a confidential intermediary may file with the Registry a birth parent's death certificate, if available (instead of a birth parent's death certificate or obituary). Makes other changes.

House Floor Amendment No. 3
In provisions setting forth the membership of the Original Birth Certificate-Access Public Information Campaign Oversight Committee, makes the following changes: deletes language stating that certain representatives of organizations shall be members; adds language stating that representatives of the organizations that serve on the Illinois Adoption Registry Advisory Council or the Confidential Intermediary Advisory Council shall be members; and adds a provision that the membership is not limited to the stated members.
