 1        AN ACT concerning health insurance.

 2        Be  it  enacted  by  the People of the State of Illinois,
 3    represented in the General Assembly:

 4        Section 5.  The  Uniform  Health  Care  Service  Benefits
 5    Information  Card  Act  is  amended by changing Section 15 as
 6    follows:

 7        (215 ILCS 139/15)
 8        Sec. 15. Uniform health care  benefit  information  cards
 9    required.
10        (a)  A  health  benefit  plan that issues a card or other
11    technology and provides coverage  for  health  care  services
12    including  prescription  drugs or devices also referred to as
13    health care benefits and an  administrator  of  such  a  plan
14    including, but not limited to, third-party administrators for
15    self-insured  plans  and state-administered plans shall issue
16    to its insureds a card or other technology containing uniform
17    health care benefit information.   The  health  care  benefit
18    information  card  or  other  technology  shall  specifically
19    identify and display the following mandatory data elements on
20    the card:
21             (1)  processor   control  number,  if  required  for
22        claims adjudication;
23             (2)  group number;
24             (3)  card issuer identifier;
25             (4)  cardholder ID number; and
26             (5)  cardholder name.
27        (b)  The uniform health care benefit information card  or
28    other  technology shall specifically identify and display the
29    following mandatory data elements on the back of the card:
30             (1)  claims submission names and addresses; and
31             (2)  help desk telephone numbers and names; and.

 1             (3)  the names of hospitals at which an insured  may
 2    obtain  medical  care  with maximum coverage under the health
 3    benefit plan.
 4        (c)  A new uniform health care benefit  information  card
 5    or  other technology shall be issued by a health benefit plan
 6    upon enrollment and reissued upon any change in the insured's
 7    coverage that affects mandatory data  elements  contained  on
 8    the card.
