| 2 | | WHEREAS, Dr. Daniel Hoffman, the son of a postman, was born | 3 | | in Henry, Illinois; he taught himself to read by memorizing | 4 | | children's books; one of his favorite books, Dr. Dan the | 5 | | Bandage Man, instilled a lifelong love for medicine at a young | 6 | | age; and
| 7 | | WHEREAS, Dr. Hoffman graduated from the University of | 8 | | Illinois at Urbana in 1974; he knew the medical profession was | 9 | | the only future for him, so he attended medical school at the | 10 | | University of Illinois College of Medicine in Peoria; and
| 11 | | WHEREAS, During his time at medical school, Dr. Hoffman met | 12 | | his future wife, Linda Sargent; their marriage in 1977 was the | 13 | | beginning of a personal and professional partnership that | 14 | | produced two sons, Doug and Eric, and a successful medical | 15 | | practice; and
| 16 | | WHEREAS, From 1977 until 1981, Dr. Hoffman was a resident | 17 | | in internal medicine at St. Francis Medical Center; he wanted | 18 | | to practice medicine and raise his family in a town small | 19 | | enough where he could get to know everyone, but still big | 20 | | enough to have a McDonald's; with encouragement from his close | 21 | | friend, Dr. Charles Longwell, he left St. Francis Medical | 22 | | Center and moved to Good Samaritan Hospital in Mt. Vernon; |

| 1 | | using his wife's training and experience as a certified medical | 2 | | office assistant and transcriptionist, they worked hard to | 3 | | establish his new practice, specializing in internal medicine | 4 | | and geriatrics; within three weeks, his schedule was full, and | 5 | | it remained that way for 37 years; and
| 6 | | WHEREAS, In 2007, Dr. Hoffman became a Certified Physician | 7 | | Executive at the American College of Physician Executives; in | 8 | | 2008, he acquired his master's in medical management from Heinz | 9 | | College at Carnegie Mellon University; and
| 10 | | WHEREAS, Dr. Hoffman and his wife contribute to many | 11 | | philanthropic organizations, including the Cedarhurst Center | 12 | | for the Arts, where Linda became a Counselor Emeritus, and they | 13 | | have both donated their time, service, and financial support; | 14 | | both have a keen interest in promoting education through | 15 | | various venues, and Dr. Hoffman served on the board of Mt. | 16 | | Vernon Township High School for seven years, including serving | 17 | | as president; for the past 18 years, he has been the SSM Good | 18 | | Samaritan Hospital's Administrative Medical Director; he also | 19 | | puts his woodworking skills to the test each year for the YMCA | 20 | | Derby Days fundraiser by donating the table centerpieces as | 21 | | well as the wooden horses used in the fundraiser; and
| 22 | | WHEREAS, Dr. Hoffman and Linda plan to spend their | 23 | | retirement cooking, cruising, playing golf, and gardening; |

| 1 | | most importantly, they are eager to spend more time with their | 2 | | children and grandchildren, including Andrew, Caleb, and a baby | 3 | | on the way; therefore, be it
| 4 | | RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE ONE | 5 | | HUNDREDTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that we | 6 | | congratulate Dr. Daniel Hoffman on the occasion of his | 7 | | retirement from practicing medicine for 37 years; we commend | 8 | | his hard work, dedication, and accomplishments and thank him | 9 | | for his contributions to his community and the field he has | 10 | | loved from a young age; and be it further
| 11 | | RESOLVED, That a suitable copy of this resolution be | 12 | | presented to Dr. Hoffman as an expression of our esteem and | 13 | | respect.
