1 AMENDMENT TO HOUSE BILL 4712
2 AMENDMENT NO. ______. Amend House Bill 4712 on page 1, line
3 17 by inserting after "entity" the following:
4 "; however, a person or entity that provides an insurance card
5 must print on the card an identification number unique to the
6 holder of the card in the format prescribed by Section 15 of
7 the Uniform Prescription Drug Information Card Act"; and

8 on page 3, line 13 by changing "2005" to "2006".
