| AMENDMENT TO HOUSE BILL 4963

| AMENDMENT NO. ______. Amend House Bill 4963 by replacing |

| everything after the enacting clause with the following:

| "Section 5. The Illinois Insurance Code is amended by |
| 5 |
| changing Section 370e as follows:
|

| (215 ILCS 5/370e) (from Ch. 73, par. 982e)
|
| 7 |
| Sec. 370e. Companies which issue group accident and health |
| 8 |
| policies
or blanket accident and health plans to employer |
| 9 |
| groups in this State shall
provide the employer with notice of |
| 10 |
| termination of a group or blanket accident
and health plan |
| 11 |
| because of the employer's failure to pay the premium when
due. |
| 12 |
| The notice shall include a statement advising the employer that |
| 13 |
| the employer may be required to notify its employees under |
| 14 |
| Section 2 of the Employee Benefit Contribution Act. |
| 15 |
| The insurance company shall send a copy of such notice to |
| 16 |
| the Department.

| Section 99. Effective date. This Act takes effect upon |
