 1                    AMENDMENT TO SENATE BILL 1983

 2        AMENDMENT NO.     .  Amend Senate Bill 1983  on  page  1,
 3    line  5,  after  "3", by inserting "and adding Section 15.5";
 4    and

 5    on  page  6,  line  4,  after  "prescription",  by  inserting
 6    "information,   including   prescriptions   for    controlled
 7    substances,"; and

 8    on  page  11,  line  16,  after  "prescription", by inserting
 9    "information,   including   prescriptions   for    controlled
10    substances,"; and

11    on  page  12,  immediately  below  line  2,  by inserting the
12    following:

13        "(225 ILCS 85/15.5 new)
14        Sec. 15.5.  Prescription information.
15        (a)  Uncoordinated  multiple  controlled  substances  and
16    drug seeking tendencies pose  a  significant  threat  to  the
17    health,  safety,  and  welfare  of patients.  To address this
18    threat,  the  General  Assembly  believes  a  physician   who
19    prescribes  controlled  substances  should  be  provided with
20    prescription information from pharmacies.
21        (b)  Upon request, a pharmacist shall provide a physician

 1    licensed to practice medicine in  all  its  branches  who  is
 2    prepared   to   prescribe  or  has  prescribed  a  controlled
 3    substance for a patient with information from  the  patient's
 4    most  recent patient profile, including information about any
 5    prescriptions for controlled substances.".
