AMENDMENT TO SENATE BILL 1620

AMENDMENT NO. ______. Amend Senate Bill 1620 on page 2, line 5, after the period, by inserting the following: "Pending approval from the resident, the facility must inform the resident's designated case coordination unit, as defined in 89 Ill. Adm. Code 240.260, of the pending discharge and must provide the resident or his or her representative with the case coordination unit's telephone number and other contact information."; and

on page 2, line 16, after the period, by inserting the following: "Pending approval from the resident, the facility must inform the resident's designated case coordination unit, as defined in 89 Ill.
