AMENDMENT TO SENATE BILL 2695

AMENDMENT NO. ______. Amend Senate Bill 2695 on page 1, line 11 by inserting after the period the following:

"With the consent of the resident or his or her representative, the facility must inform the resident's designated case coordination unit, as defined in 89 Ill. Adm. Code 240.260, of the resident's pending discharge and must provide the resident or his or her representative with the case coordination unit's telephone number and other contact information."; and

on page 1, by replacing lines 17 through 23 with the following: "removal under Section 3-422.".
