3 lines 15 and 16 by replacing "the Illinois Hospice & Palliative
4 Care Organization" with the following:
5 "a statewide organization whose primary membership consists of
6 hospice programs"; and

7 on page 2, line 26 and page 3, line 1 by replacing "the
8 Illinois Hospice & Palliative Care Organization" with the
9 following:
10 "a statewide organization whose primary membership consists of
11 hospice programs"; and

12 on page 3, lines 3 and 4 by replacing "the Illinois Hospice &
13 Palliative Care Organization" with the following:
14 "a statewide organization whose primary membership consists of
15 hospice programs"; and
