4 on page 5, line 3, by replacing ", ~~or~~" with "or"; and

5 on page 5, line 4, by deleting ", or other agreement"; and

6 on page 6, line 1, by deleting "; or other agreement"; and

7 on page 6, by replacing lines 3 and 4 with the following:
8 "installment payment contract or lease purchase agreement with
9 a qualified provider or with a third-party".
