AMENDMENT TO SENATE BILL 658

2     AMENDMENT NO. ______. Amend Senate Bill 658 by replacing
3 everything after the enacting clause with the following:
 
4     "Section 5. The Alzheimer's Special Care Disclosure Act is
5 amended by changing Sections 10 and 15 as follows:
 
6     (210 ILCS 4/10)
7     Sec. 10. Facility defined. As used in this Act, "facility"
8 means a facility licensed or permitted under the Nursing Home
9 Care Act, the Life Care Facility Act, the Assisted Living and
10 Shared Housing Act, the Community Living Facilities Licensing
11 Act, or subsection (a-20) of Section 30 of the Alternative
12 Health Care Delivery Act, or a facility designated as a
13 supportive living facility under Section 5-5.01a of the
14 Illinois Public Aid Code.

1     (210 ILCS 4/15)
2     Sec. 15. Disclosure requirements. A facility that offers to
3 provide care for persons with Alzheimer's disease through an
4 Alzheimer's special care unit or center shall disclose to the a
5 State agency responsible for licensing or permitting the
6 facility and or to a potential or actual client of the facility
7 or such a client's representative the following information in
8 writing on request of the Agency or client:
9         (1) the form of care or treatment that distinguishes
10     the facility as suitable for persons with Alzheimer's
11     disease;
12         (2) the philosophy of the facility concerning the care
13     or treatment of persons with Alzheimer's disease;
14         (3) the facility's pre-admission, admission, and
15     discharge procedures;
16         (4) the facility's assessment, care planning, and
17     implementation guidelines in the care and treatment of
18     persons with Alzheimer's disease;
19         (5) the facility's minimum and maximum staffing
20     ratios, specifying the general licensed health care
21     provider to client ratio and the trainee health care
22     provider to client ratio;
23         (6) the facility's physical environment;
24         (7) activities available to clients at the facility;
25         (8) the role of family members in the care of clients

1     at the facility; and
2         (9) the costs of care and treatment under the program
3     or at the center.
