AMENDMENT TO SENATE BILL 40

AMENDMENT NO. ______. Amend Senate Bill 40 on page 13, line 3, after "shall", by inserting ", upon request,"; and

on page 13, line 7, after "promptly", by inserting ", within 10 days before the hearing,"; and

on page 17, line 8, after "minimum", by inserting ", which shall be reviewed by the Board within 120 days"; and

on page 17, line 9, after "service", by inserting "within an existing healthcare facility"; and

on page 17, line 10, after "service", by inserting "within an existing healthcare facility"; and

on page 23, by replacing lines 5 through 7 with the following: "holder submitting the required information and reports. Beginning 6 months after the effective date of this amendatory Act of the 97th General Assembly, the Board shall notify, in writing, a permit holder of the due date for the post-permit requirements no later than 30 days before the due date for the requirements.".
