AMENDMENT TO SENATE BILL 1853

2    AMENDMENT NO. ______. Amend Senate Bill 1853, AS AMENDED,
3with reference to page and line numbers of Senate Amendment No.
42 as follows:
 
5on page 2, line 12, by replacing "authorities" with
6"authority's"; and
 
7on page 5, line 13, by replacing "container" with "container,";
8and
 
9on page 5, line 16, by replacing "liner" with "liner,"; and
 
10on page 9, line 20, by replacing "authorities" with
11"authority's"; and
 
12on page 10, line 4, by deleting "Attorney General or by the";

3    (410 ILCS 535/11)  (from Ch. 111 1/2, par. 73-11)
4    Sec. 11. Information required on forms.
5    (a) The form of certificates, reports, and other returns
6required by this Act or by regulations adopted under this Act
7shall include as a minimum the items recommended by the federal
8agency responsible for national vital statistics, subject to
9approval of and modification by the Department. All forms shall
10be prescribed and furnished by the State Registrar of Vital
11Records.
12    (b) On and after the effective date of this amendatory Act
13of 1983, all forms used to collect information under this Act
14which request information concerning the race or ethnicity of
15an individual by providing spaces for the designation of that
16individual as "white" or "black", or the semantic equivalent
17thereof, shall provide an additional space for a designation as
18"Hispanic".
19    (c) Effective November 1, 1990, the social security numbers
20of the mother and father shall be collected at the time of the
21birth of the child. These numbers shall not be recorded on the
22certificate of live birth. The numbers may be used only for
23those purposes allowed by Federal law.
24    (d) The social security number of a person who has died

1shall be entered on the death certificate; however, failure to
2enter the social security number of the person who has died on
3the death certificate does not invalidate the death
4certificate.
5    (e) If the place of disposition of a dead human body or
6cremated remains is in a cemetery, the burial permit shall
7include the place of disposition. The place of disposition
8shall include the lot, block, section, and plot or niche, and
9depth, if applicable, where the dead human body or cremated
10remains are located. This subsection does not apply to cremated
11remains scattered in a cemetery.
